Physicals and Medical Clearances
Please be advised that physicals are valid for one (1) year. No student is allowed to try out or participate in the pre-season of any sport without having a valid physical on record. Please check with the Athletic Trainer if you are unsure of when your physical expires. It is the student-athlete’s responsibility to make sure their physical is valid.
In order to play each season, the CIF-NCS requires that each student-athlete fills out an updated Health History. Justin-Siena also requires an updated Athlete Information Form, which includes emergency contact information, medical alerts, and insurance information. These forms will be filled out on SportsWare.
These steps are required for your child’s participation in athletics.
- Please register your son/daughter on SportsWare, as per the instructions.
- The physical portion of the medical paperwork should be uploaded to SportsWare per the instructions. We are a paperless sports medicine program and will not collect paper physical forms.
- Once reviewed, you will be alerted if your child is cleared to participate in activities. You will also be notified if he or she is not cleared and be given a reason.
- You must take the ImPACT baseline test. Please refer to ImPACT Testing Instructions for a list of sports required to take ImPACT and how to complete the test. Submission of health information by August 1st, is vital so that preparations can be made in time for the first day of school and athletics. - PHYSICALS – We are going digital! All Physical Forms MUST be uploaded into SportsWare (see instructions). No paper physical forms will be accepted for the 2019-2020 school year. Ms. Goble will review all medical paperwork, ensuring proper documentation and compiling a list of eligible student-athletes each season. No student may try out or participate in pre-season workouts for a sport before submitting the proper medical forms prior to the beginning of school and each athletics season. Parents and student-athletes please check-in with Ms. Goble to ensure you are cleared for athletics.
- REMEMBER: You must take the ImPACT baseline test. Testing opportunities will be announced prior to each season. It is the responsibility of the student-athlete to attend a testing session or contact Ms. Goble to schedule a testing session. Please refer to the ImPACT Testing Instructions. All medical paperwork that your child will need is located on SportsWare. Please complete your son/daughter’s SportsWare account in order to download the forms. Each form is reviewed by the athletic trainer to ensure that all paperwork is correctly completed. To register, please see below:
